Patient medical records
You may request a copy of your medical record by your BJC Medical Group provider. All requests are processed by our Health Information Management Release Services.
To request a copy of your medical records by mail, please download the appropriate form:
Mail fully completed and signed form(s) back to your BJC Medical Group provider’s office OR fax form(s) to 314-996-7008.
For access to a deceased patient’s medical records
Authorized Relative Certification: Submit a copy of the medical records request form and a copy of the death certificate along with completed form.

If you need hospital records from one of the below hospitals, you will need to request them directly from the hospital by following the links below.

Fees
BJC Medical Group partners with CIOX Health, the nation's largest provider of Release of Medical Information services, to process and fulfill your request for a copy of your medical record. For patient personal use or third-party requests, we may charge a reasonable fee for copying requested health information. View fee chart »
There is no charge for records sent directly to another physician.
BJC Medical Group will respond to your request for health information within 30 days of our receipt of your request. If, however, your health information is not readily accessible by BJC Medical Group or is maintained in an off-site storage location, we have an additional 30 days to respond to your request. If we require additional time, we will contact you to inform you of this time extension.

If you have already submitted a medical records request, and it has been at least seven days since you sent in the request, please contact CIOX Health at 314.778.3310 to check the status of your request.
Right to amend your health information
If you believe there is an error in your records, you can request that we correct them. We will review the information, as requested, and either make the correction or let you know why we think our information is correct.
